What is TSS? Toxic Shock Syndrome explained
Toxic Shock Syndrome (TSS for short) is often known as tampon disease. Yet it can affect not only women but men too – and tampons are far from the only possible cause.
What is TSS?
TSS is an infectious disease that is actually very rare, but it causes sudden, life-threatening complications. Left untreated, TSS leads to multiple organ failure. The insidious thing is that the germs responsible are found almost everywhere. Around 15–40 per cent of all healthy people carry them in their nose and throat. The pathogens can produce certain toxins, to which our immune system responds by forming antibodies.
If the germs enter the body in large numbers and the immune system has not yet formed antibodies, it is unable to neutralise the germs and toxins.
Who is affected by TSS?
Toxic Shock Syndrome mainly affects younger people whose immune systems are not yet fully developed. By adulthood, more than 90 per cent of people have the corresponding antibodies.
Women, men and even children can develop TSS, but women between the ages of 10 and 50 are at a higher risk than men of the same age. The pathogens enter the body most easily through skin wounds, burns or insect bites.
For people who menstruate, it can therefore also occur when using tampons, menstrual cups or a diaphragm, as well as through bacterial infections and complications such as an abscess.
TSS symptoms
Signs of TSS: watch out for these 8 symptoms
The typical symptoms of TSS are:
- sudden headaches
- dizziness
- vomiting
- a drop in blood pressure
- high fever
- chills
- muscle pain
- drowsiness
Some people also report a sunburn-like rash. If you notice these symptoms, please contact your doctor immediately for a thorough check-up.
If it really is Toxic Shock Syndrome, every minute truly counts – so play it safe and get checked by a doctor if you have any suspicion.
One of the best-known TSS cases comes from the USA, where the American model Lauren Wasser lost both legs to TSS. The case shook the entire feminine hygiene world. Lauren sued the company at the time, as its tampons were made from synthetic materials and triggered Toxic Shock Syndrome in her. But she didn't just take the big tampon manufacturers to court – she also campaigned for the Robin Danielson Act.
The Robin Danielson Act was named after a woman who died of TSS in the 1990s. The act is intended to require manufacturers of period products to list all ingredients on the packaging. Sad fact: the act has still not come into force to this day.

Are tampons dangerous?
How dangerous are tampons and how can you prevent TSS?
The link between TSS and tampons was established in the USA in the 1980s, when highly absorbent tampons came onto the market. Thanks to their higher capacity, these were designed to be left in the body for longer.
During this time, cases of TSS rose noticeably. The warm, moist environment of the vagina encourages the growth of bacteria and fungi. Most of them simply live there without causing any problems. A tampon, however, offers the perfect surface for them to multiply. The immune system struggles to act against the bacteria in the tampon, because they are cut off there from the rest of the body.
It becomes dangerous when the toxin produced by the bacteria enters the bloodstream. Depending on the state of the immune system, the body may then be unable to neutralise this toxin. Left untreated, this can become life-threatening and lead to multiple organ failure.
Contrary to popular belief, the risk of developing Toxic Shock Syndrome from using tampons or menstrual cups is low. The RKI (Robert Koch Institute) reports 3–6 cases per 100,000 women per year, 92 per cent of which are linked to menstruation.
How can I prevent TSS?
Hygiene, hygiene, hygiene – how to prevent TSS
Careful hygiene during your period is essential, so keep the following in mind when using tampons or a menstrual cup:
- Wash your hands thoroughly before and after insertion
- Start with the smallest tampon size / menstrual cup for your first period
- On heavy-flow days, change your tampon / empty your menstrual cup more often
- Change your tampon / empty your menstrual cup after 8 hours at the latest
- Always clean your menstrual cup thoroughly and boil it at the end of your period

As an alternative to menstrual cups or tampons, you can of course also switch to pads or panty liners. While these cannot rule out TSS entirely, they do reduce the risk of infection.
