Endometriosis vs PCOS: The Differences Explained – and What Helps in Everyday Life
By Elvira Greblic · 1 October 2026
It’s something far too many people with a uterus have heard at some point: “Period pain is completely normal – you just have to get through it.” But when the pain becomes unbearable every month, your cycle is completely unpredictable or your period stops altogether, there’s often more going on.
Two of the most common – and yet most frequently confused or belatedly diagnosed – gynaecological conditions are endometriosis and polycystic ovary syndrome (PCOS). Although both conditions can have a major impact on the menstrual cycle, their causes are fundamentally different.
What to expect in this article:
- We explain clearly what endometriosis and PCOS actually are.
- You’ll get to know the typical symptoms of both conditions.
- We show why they are often confused on the way to a medical diagnosis.
- You’ll find out how to make your everyday life gentler and more comfortable despite severe cycle-related symptoms.
At Selenacare, we want to inform, break taboos and show you that you are not alone with your symptoms. Do conventional sanitary products add to your stress on heavy days? Discover our pressure-free, leak-proof period underwear for gentle, reliable protection – no matter how unpredictable your cycle is!
What is endometriosis?
With endometriosis, tissue similar to the normal lining of the uterus grows where it shouldn’t – outside the uterus, for example on the ovaries, the peritoneum or the bowel. This tissue responds to the hormones of the cycle: it builds up cyclically and bleeds. However, because the blood cannot drain away from the abdominal cavity, this often leads to inflammation, adhesions and the formation of cysts.
Typical symptoms of endometriosis include:
- Severe, cramping lower abdominal pain, often (but not always) directly linked to your period.
- Very heavy menstrual bleeding (hypermenorrhoea).
- Pain during sex or when going to the toilet.
Many people with endometriosis also report that internal period products such as firm menstrual cups or tampons often make the pain worse, as they put pressure on the highly sensitive pelvic floor or create suction.
What is polycystic ovary syndrome (PCOS)?
Unlike endometriosis, PCOS is primarily a hormonal and metabolic disorder. There is often an excess of so-called androgens (male hormones), which disrupts the normal function of the ovaries. As a result, eggs do not mature properly and ovulation happens less often or not at all. Typical symptoms of PCOS include: Very irregular cycles, very long cycles or no periods at all for several months. Many small follicles on the ovaries, which look like a string of pearls on an ultrasound scan. Physical changes such as acne, increased body hair (e.g. facial hair) or hair loss on the top of the head. Often accompanying insulin resistance, which can lead to metabolic problems or weight gain. For people with PCOS, the biggest everyday challenge is often the emotional strain of unpredictability. If you never know exactly when your next bleed will start, you live in a constant state of tension and alertness.
What they have in common: why they are often confused
Although their biological causes are different, PCOS and endometriosis share some symptoms that often make the road to a diagnosis a long one:
- Fertility challenges: Both conditions are among the most common causes of difficulty conceiving in women of reproductive age.
- Cycle chaos: Irregular cycles occur with both.
- Acyclic symptoms: Both conditions can also cause vague exhaustion, mood swings or a dragging pain in the lower abdomen outside of your period.
How Selenacare makes your everyday life gentler
We can’t cure hormonal imbalances or endometriosis lesions. But as experts in period products, we know that the right protection during your period can contribute enormously to your physical well-being and help reduce stress.
- Pressure-free protection with endometriosis: When tampons put extra strain on an abdomen that already hurts, period underwear offers a gentle, purely external solution with no foreign-body sensation. Our Selenacare period briefs sit softly without digging into your tummy, so your pelvic floor muscles can relax more easily.
- Peace of mind with PCOS cycles: When your cycle is unpredictable, our lightweight period briefs give you a huge sense of security in everyday life. They are as discreet to wear as normal underwear, yet reliably absorb unexpected bleeding. So you are always protected.
- Gentle on your intimate flora: Hormonal fluctuations often make the intimate area more sensitive. Our washable textiles contain no irritating plastic films or artificial fragrances.
Important: Your pain and cycle problems are valid. If you suspect you may have endometriosis or PCOS, take your body’s signals seriously and see your gynaecologist for an accurate diagnosis and treatment options.
