Abscess in the Intimate Area: Causes, Treatment & When to See a Doctor
By SELENACARE Redaktion · 22 September 2026
A painful lump, a throbbing swelling or an abscess on the labia: many women know this unpleasant feeling, but rarely talk about it out of embarrassment or uncertainty. Yet an abscess in the intimate area is surprisingly common. Shaving, friction and moisture make the sensitive area around the vulva and vaginal opening particularly prone to small inflammations, which can develop into an encapsulated collection of pus.
What to expect in this article:
- Causes & symptoms: How an abscess in the female intimate area develops and how to recognise it.
- Recurring abscesses: Why conditions such as acne inversa are often behind them – and why this has nothing to do with poor hygiene.
- First aid & gentle home remedies: What really helps with mild cases and in the early stages.
- Warning signs & seeing a doctor: How the medical incision is carried out painlessly and when you need to act immediately (including what a red streak means).
- Prevention: How to protect your skin from renewed irritation with breathable textiles.
Trapped heat and friction from synthetic pads can put extra strain on irritated skin in the intimate area. Treat your skin to something good: discover our breathable period underwear as well as soft washable pads and panty liners from Selenacare for a feeling that is always dry, gentle and irritation-free!
What is an abscess in the intimate area and how does it develop?
An abscess in the intimate area is a firm, encapsulated collection of pus in the tissue. It usually develops when bacteria – such as staphylococci or E. coli – enter deeper layers of tissue through tiny skin injuries. The immune system fights the germs and walls off the inflammation.
Typical symptoms:
- A firm, very painful or throbbing swelling on the labia, the perineum or at the vaginal opening.
- Red, warm skin that is extremely tender to pressure.
- Pain when walking, sitting or when clothing touches the area.
- Occasionally a visible yellowish head of pus.
Common triggers:
- Intimate shaving & ingrown hairs: Microscopic cuts or inflamed hair follicles (folliculitis) are the most common entry point for germs.
- Blocked Bartholin’s glands: A blockage of the gland ducts at the vaginal opening leads to a painful swelling (an abscess on the labia, or bartholinitis).
- Friction & trapped sweat: Tight clothing and non-breathable disposable pads create a warm, moist environment in which bacteria multiply perfectly.
Recurring abscesses: a topic we need to talk about
If an abscess on the labia or in the groin keeps coming back, it often leaves those affected feeling desperate. Unfortunately, the myth that recurring abscesses in the intimate area are caused by poor hygiene is stubbornly persistent. This is medical nonsense and causes those affected enormous emotional harm!
If abscesses in the intimate area keep recurring, an as yet undiagnosed skin condition called acne inversa (hidradenitis suppurativa) may be behind them. It affects at least one per cent of the population. In this condition, the hair follicles and sweat glands in skin folds become inflamed again and again, leading to painful lumps, abscesses and sinus tracts. If you regularly suffer from inflammation like this, make sure you see a dermatologist or gynaecologist who is familiar with acne inversa.
Gentle help for mild cases & early stages
If the area is only slightly red, you can’t yet feel a hard lump of pus, or you would like to gently support the healing process, the following measures can help:
- Never squeeze or lance it yourself! This is the most important rule. If you try to squeeze an abscess in the vagina or on the labia yourself, the capsule can burst inwards. The bacteria can then get deep into the tissue or into the bloodstream, which can lead to serious infections.
- Warm, moist sitz baths & compresses: A pleasantly warm (not too hot!) sitz bath with liquid chamomile extract (e.g. Kamillosan) or sea salt soothes irritated skin, has an anti-inflammatory effect and boosts blood circulation. Alternatively, you can soak a clean cotton cloth in cooled chamomile tea and place it gently on the area for 10 to 15 minutes.
- Drawing ointments (ichthammol): For superficial, closed spots on the outer labia or in the groin, over-the-counter drawing ointments (shale oil ointments) can help draw the inflammation to the surface of the skin and soften the tissue.
Important: Drawing ointments may only be applied to the outer, hair-bearing skin and never directly to the sensitive mucous membranes or to open wounds! - Antiseptic wound gels & zinc ointment: An alcohol-free, disinfecting wound gel (e.g. Octenisept Gel) keeps the area low in germs and provides moisture. Zinc ointment helps to keep moisture away and reduce friction on the inner thighs.
- Avoid pressure: Wear loose cotton underwear and loose-fitting trousers so that nothing rubs against the inflamed area.
When should you see a doctor straight away?
An abscess in the female intimate area should not be taken lightly. See a gynaecologist or your GP promptly if:
- the pain is very severe or throbbing, or the lump is growing quickly.
- accompanying symptoms such as fever, chills or generally feeling unwell occur.
- the abscess is located right at the vaginal opening or keeps coming back.
- you notice a red streak spreading from the swelling: This is a fine, pink to dark red line on the skin that starts at the abscess and slowly extends further (often towards the groin, where the lymph nodes are located). This streak is often warm, slightly raised or tender to pressure. It is a clear warning sign of inflammation of the lymphatic vessels (lymphangitis) – commonly referred to as "blood poisoning". If you notice a red line like this on your skin, you should see a doctor or go to A&E that same day!
